What is the Housing Support Program?
Beginning July 1, 2025, Medicaid will pay for some housing-related services for eligible individuals enrolled in NJ FamilyCare/Medicaid. The Housing Support Program provides access to housing support services that help homeless or housing-insecure individuals find, obtain and maintain stable housing, which ultimately improves health outcomes. With this assistance, the integration of NJ FamilyCare/Medicaid funding and care coordination through managed care organizations will seamlessly deliver whole-person care while enhancing existing funding programs for the housing insecure.

- Pre-tenancy and tenancy providers: If you offer move-in supports and are interested in becoming a participating provider, you can start the approval process by completing a credentialing application. For more information, please visit the Camden Coalition website.
Completed applications can be sent by email to hcbsprovidernetwork@uhc.com. In the subject line, please state the organization’s name, Housing Supports – Application Submission.
