Out-of-network TIN verification

If you need to submit an out-of-network medical claim, or you have received a letter requesting information to verify provider billing, you can start the process of registering your tax ID number (TIN) now.

Be sure to have a PDF version of your W-9 tax form and documentation to prove your primary practice location address.

Out-of-network facility

If you have not worked with UnitedHealthcare in the past please follow this process:

 

  1. Submit - Submit a paper claim with your W-9 attached using the submission information from the member ID card.
  2. Register - Once the claim is processed, register for the UnitedHealthcare Provider Portal.

Once you have completed steps 1 and 2, register for the portal to:

  • Estimate and manage claims and payments
  • Check prior authorizations
  • Chat with an advocate for help
