Ensure uninterrupted patient care: Help your Medicaid patients avoid a lapse in coverage
Your UnitedHealthcare Community Plan of New York (Medicaid) patients may need to renew their coverage. Affected members should have received a redetermination packet from The NY State of Health, notifying them that they need to reapply (this typically happens every 12 months). If a patient misses the renewal date, their coverage may end.

Renewing helps ensure these members can continue to receive services through your office. Please remind your patients to complete and return their redetermination packet as soon as possible.
