New York Medicaid: Update your data in My Practice Profile or CAQH
When you attest or update your demographic information in My Practice Profile or CAQH, you can help ensure members and other health care professionals can find your practice easier and faster.
As a health care professional contracted with UnitedHealthcare Community Plan of New York, you’re also required to verify the accuracy of your demographic information every 90 days.
Keeping your data accurate helps our members find you. Our members make most of their care decisions based on:
- New patient acceptance
- Office location
- Services and treatments provided
How to verify your data
Please use one of the following methods to verify your data:
- My Practice Profile, located in the UnitedHealthcare Provider Portal, lets you verify and attest to the accuracy of your practice’s data
- To access My Practice Profile, go to UHCprovider.com and select Sign In in the upper right corner
- Sign in with your One Healthcare ID and password
- If you need to set up a One Healthcare ID, visit New User & User Access

- CAQH ProView® is a multi-payer solution that allows you to update your profile in real time and have instant access to directory services
- Existing users: Log in to your CAQH ProView
- New users: Go to CAQH ProView to create a new, secure account and review the CAQH step-by-step video and user guide
If you’re not registered with My Practice Profile, use our practice change or demographic change form to make these updates.
