Massachusetts: Expedited credentialing for care providers of closed hospitals
You can now get expedited credentialing to join our network if you’re a health care professional previously associated with Carney Hospital or Nashoba Valley Medical Center. UnitedHealthcare Community Plan of Massachusetts will also quickly modify information in your existing credentialing file.

We’re offering expedited credentialing to help you transition from either of these 2 facilities that were abruptly closed.
How to submit your request
Please submit your credentialing request using our Onboard Pro tool on the UnitedHealthcare Provider Portal.
- Go to UHCprovider.com to sign in at the top-right corner using your One Healthcare ID and password. If you don’t have one go to UHCprovider.com/access.
- Enter your entity details including tax ID numbers (TINs), legal name and contact information
- Complete the Council for Affordable Quality Healthcare (CAQH) registration, if required
- Make sure attestations and documentation are current
For demographic or roster updates, UnitedHealthcare is encouraging impacted health care providers to:
- Use either My Practice Profile or CAQH Provider Data Portal to submit your demographic data updates. For more information, see Provider data updates and attestation.
- Submit your delegated roster to your roster manager
- Submit non-delegated providers added to groups using Onboard Pro
