Trellus Elevate: Empower your patients with personalized IBD support
Trellus Elevate™ is a no-cost virtual program that helps patients proactively address daily disruptive symptoms and address the emotional, psychological and nutritional aspects of living with IBD. The program is available to eligible members who have been diagnosed with IBD and have either the UnitedHealthcare commercial plan or Oxford plan in New York or Connecticut at no additional cost.
If you have a UnitedHealthcare patient who you think might benefit from the program, please encourage them to enroll by Sept. 30, 2024.
