Submit your 2023 diagnostic data for ACA-covered plans
We’re committed to helping improve the quality of care provided to your patients. Part of that commitment is collecting diagnostic data for patients enrolled in certain small-group and individual Affordable Care Act (ACA)-covered health plans.
What this means for you
We’ll be working with you to facilitate a medical record review of 2023 dates of service for some number of patients enrolled in certain ACA-covered health plans. While this data collection is meant to help support the care you give your patients, it’s also required by the Department of Health and Human Services.
How to submit
We’ve mailed you directions on the requirements and the return process. We’ve also engaged Optum and Ciox Health to conduct these medical chart reviews. A Ciox Health representative will work with you to obtain:
- Retrieval options
- A list of the requested patients’ medical records for services rendered by dates of service listed on the pull list
You’ll also need to work with your advocate account manager to make sure they know:
- The correct contact for chart collection
- How you usually submit charts
If you haven’t received the instructions, they may still be in transit. If you haven’t received them by the end of the month, please call 877-442-4143.
