Prospero and Landmark combine to create one home-based care program
Between Jan. 1 and July 30, 2023, Landmark Health and Prospero Health, 2 UnitedHealthcare complex care and palliative care management programs, are joining to create one program that will continue to operate under Landmark Health.
Who this change affects
This change affects qualified members who have UnitedHealthcare® Medicare Advantage health plans. We’ve sent letters to participating Prospero program members informing them of their respective market’s upcoming launch date. See below for which areas are affected in 2023:

Who can participate
Members are qualified for the program by meeting clinical criteria focused on chronic conditions. Landmark collaborates with primary care providers to offer the following in-home care services for the management of chronic conditions and geriatric syndromes:
- Urgent episodes of care
- Post-acute episodes of care
- Palliative care
- Behavioral care
- Medication management
- Social drivers of health
