Effective Jan. 1, 2023, pregnant and postpartum patients enrolled in Medicaid have an opportunity to participate in the Maternal Opioid Misuse (MOM) model through the Maryland Department of Health.
Funded by the Centers for Medicare & Medicaid Services (CMS) Innovation Center, the MOM model works collaboratively with Medicaid managed care organizations (MCOs) to provide enhanced case management services that focus on improving care for pregnant and postpartum patients diagnosed with opioid use disorder (OUD).
You can help support your patients who are potential and/or enrolled MOM participants through the following:
Dedicated MOM case managers will create individual care plans for MOM participants, including a minimum of 1 health care visit every month. Primary care providers can review their patients’ care plans and find the name and contact information of the MOM case manager through CRISP, the designated health information exchange system for Maryland.
