Overview

We’ve created this reference guide of resources to help primary care providers screen patients for depression, Attention Deficit Hyperactivity Disorder (ADHD) and alcohol/substance misuse. As you’re aware, treatment is critical since it can contribute to the patient’s readiness to change at risk behaviors. 

Screening Tools and Resources

Using screening tools in primary care follows the recommendations of the U.S. Preventive Services Task Force, an independent group of national experts in prevention and evidence-based medicine for behavioral health disorders.

Depression

  • Patient Health Questionnaire (PHQ-9)*: This site includes downloadable PHQ screeners in several languages to help accurately diagnosis and track improvement in treating depression. Background information about the screening tools and scoring instructions is also available.

United Behavioral Health Support

To help support you further, United Behavioral Health, which manages behavioral health services for UnitedHealthcare members, has information to support you and your patients in treating depression, alcohol/substance use disorders or ADHD. United Behavioral Health’s website includes access to the screening tools listed in this guide, and are available to you at no cost.

United Behavioral Health also offers access to a Prevention Center of resources for depression, alcohol/substance use disorders and ADHD, which includes:

  • A library of articles to support prevention and recovery for each condition
  • Information about co-morbid conditions
  • Web links to nationally recognized practice guidelines
  • A printable self-appraisal for the patient to complete or for you to use if you refer them to a behavioral health practitioner
  • List of support resources for practitioners, patients and their families.

Behavioral Health Referrals

To refer a member to a United Behavioral Health network practitioner for assessment and/or treatment, call United Behavioral Health at the number on the back of their UnitedHealthcare member identification (ID) card.
