Relate Platinum now available on Preferred Formulary for Medicare Advantage patients

You’ve talked, we listened! Thank you for your feedback in this year’s provider survey. With those results combined with the ongoing feedback we’ve collected this year, we have some great news to share.

We’ve added Relate® Platinum products to the formulary for Preferred Individual Medicare Advantage program plans on Aug. 28, 2024. These products are now selectable in the UnitedHealthcare Hearing Provider Portal.

Please take a moment to review the updated Appendix 1 Professional Fee Schedule with the new product offering and professional fees for Preferred Individual Medicare Advantage program plans, effective Aug. 28, 2024. The updated Fee Schedule is now posted on our Provider Portal.
